First visit to the Child and Adolescent Neuropsychiatry Clinic

A 16-year-old male patient, treated until last year in the mental health facilities in another region, was referred for an outpatient evaluation. Following a recent transfer to our city, he came to our outpatient clinic for the first time, accompanied by his mother. The boy is sitting in the waiting room and looking in front of him without any emotion.

"George, can you wait here, please?" you ask the boy.

He doesn't react, so you turn to his mother. "Come with me, please, Mrs. Smith."

The mother is upset. Her son's condition bothers her greatly.

"What was George like when he was a little boy, playing with his peers?" you ask.

"Self-conscious. Especially shy and timid. It's been getting worse since he started high school," says George's mother, starting to cry.

You offer her a box of tissues.

She tells you how George gradually reduced his relationships with his peers. "He told me that he didn't like being made fun of behind his back. He kept shutting himself away from the world alone in his room," explains the boy's mother.

"Did he have any special interests?"

"Yes, he started reading a lot. At first, I was happy about it. He was interested in the history of the twentieth century. But then he started to develop a special interest in Nazism," the mother recalls and starts to cry again.
